Tammy Rainey, APRN will be leaving the practice (last day July 22nd) and moving out of state. Our plan is to reallocate all patients with Rainey as their PCP to a new provider at Appletree Bay. If you have a preference, please call the office and let Jon or Judy know.
If you have an urgent need that cannot wait until the next business day please call our main office number at 802-863-1313 and you will be transferred to an out of hours service that can contact a provider.
