Appletree Bay Primary Care has earned Patient-Centered Medical Home status from the Agency for Healthcare Research and Quality. This means that in addition to providing primary care, we have met national standards for providing and coordinating high quality primary care.
Appletree Bay Primary care is also a designated Age Friendly Health System by the Institute for Healthcare Improvements. Age-Friendly Health Systems aim to follow an essential set of evidence-based practices; cause no harm; and align with what matters to older adults (65+) and their family caregivers.
We also ensure that our patients remain up-to-date on routine screenings and services such as mammography, colonoscopy, appropriate laboratory monitoring, immunizations, and physical examinations. The medical home model recognizes that patients must be active participants in their health care for best outcomes.

Referrals for imaging, specialty care, physical therapy, mental health and substance abuse treatment
