Becoming a New Patient
- Fill out New Patient Paperwork
- Fill out the release of information so that we can get information from your previous primary care provider’s office if accepted as a new patient. Use the generic release of information form for all primary care offices except Community Health Centers (CHC), for which you should use the CHC-specific release of information form.
- Please send completed new patient paperwork and release of information form (if applicable) via email to AppletreeBayPrimaryCare@uvmhealth.org or via mail to:

- Providers will review completed forms to make sure the practice will be able to meet your needs.
- Once approved the office will send the release of information form to get previous medical records and scheduling will call you to schedule a new patient visit. We make every effort to obtain your medical records from your previous healthcare provider prior to your first visit.
- Please sign up for MyChart before your first appointment.
- Arrive 15 minutes early to your new patient visit to be checked in
- Please bring an insurance card and proof of COVID-19 vaccinations to new patient visits
